Diabetes Management Program

According to the American Diabetes Association, 18.2 million people or 6.3% of the U.S. population have diabetes, and about one-third of them aren't even aware of it.

Diabetes can't be cured, but it can be controlled.

Our Diabetes Self-Management Education Program is designed to help you manage your condition and keep you as healthy and active as possible. You'll meet in a class setting with our nurse educator to develop a self-care program. Talk to your doctor first. Then call 535-7000 to register. The 10-hour program includes an individual diabetes assessment followed by a series of four classes. Sessions cover:

  • diabetes overview
  • goal setting
  • nutrition and exercise
  • medication
  • self blood glucose monitoring
  • long-term complications
  • psycho-social concerns

A schedule will be developed for you. Diabetes education sessions are covered in many cases. Your physician may refer you by calling (808) 535-7000.

What to bring to your visit?

  • A list of all your medications
  • Any papers your doctor has given you
  • Your blood sugar testing equipment

Free Glucose Screening

Screening is provided free at Pearlridge Center, Uptown. It is valuable in identifying unknown cases of diabetes. It's also beneficial for diabetics who need assistance with ongoing monitoring.

Screening offered on the first Monday and third odd Monday of each month (e.g. Jan. 2 & 23, 2006), 7:30 – 9:30 a.m.

Diabetes Support Group

This free support group provides patients with a chance to support each other as they deal with the challenges of living with diabetes. Diabetes patients and their families can learn from experts as well as fellow group participants. Meets on the last Tuesday of each month, except holidays, 5:30-7 p.m. in the Physician's Dining Room, located on the first floor at Kapi'olani Medical Center at Pali Momi. Please leave a message at 485-4519 if you plan to attend. Led by Kapi'olani Diabetes Nurse Educator.
